If you'd rather have a name change kit made for you, send us $15 through the secure Paypal link below and we will print out all the forms and mail them to you. You can also mail us a cashier's check or money order via snail mail (contact us for the mailing address).

After receiving payment, we will email you to let you know that your order is in process, and you should receive a package in the mail within 1 week.
